Remind Participants to Return Statement of Health Forms Without Delay

Statement of health (SOH) forms required for life insurance benefit changes and additions made during annual enrollment were mailed directly to employees’ homes. Participants must return these required forms directly to MetLife at:

Employees can also fax the SOH form to MetLife at 859.225.7909.

Benefits administrators can help avoid delays by:

  • Reminding their employees to return their forms directly to MetLife
  • Checking the status of employee elections by viewing the employee’s benefit profile.

What happens if a request is “closed for no action”?
If a request is “closed for no action” after the allotted window, NRECA will notify the benefits administrator through the Message Center. Participants will receive a letter from MetLife. The “closed for no action” update will occur if MetLife:

  • Never received a completed SOH form from an employee
  • Requested more information to support a submitted SOH and the information wasn’t received
