New, Updated Beneficiary Designation Form Available
The NRECA Beneficiary Designation Form has been updated. The new form is posted on the Employee Benefits website on the Beneficiaries page for benefits administrators in the BA Guide and for participants in the Education & Resources section. Please use this form (GRPFM20047) going forward to make and update beneficiary designations for retirement, life and accidental death and dismemberment insurance and business travel accident insurance benefits.
As a reminder, there are several ways in addition to the beneficiary designation form for benefits administrators and participants to make and update beneficiary designations through the NRECA Employee Benefits website. Benefits administrators can use the Designate Beneficiary, New Hire/Rehire, Marriage, New Child, Separation or the Annual Enrollment events, as appropriate. Participants can use the online Annual Enrollment event.
In all cases, beneficiary updates must be signed by the participant and maintained by the cooperative. If the update is made online, the confirmation page must be printed and signed by the participant and kept by the co-op. If designations are made using a form, such as the NRECA Beneficiary Designation Form, it must be signed by the participant and kept by the co-op.
