Have You Sent Your 2014 SPD and SMM Distribution Acknowledgment Forms?

March 1 was the deadline to give out all summary plan description (SPD) and summary of material modifications (SMM) documents for your NRECA group benefits plans. This requirement included distributing the SMMs for medical, dental, vision, short-term and long-term disability and business travel accident insurance issued in February.
After you distribute the notices, fill out the SPD and SMM distribution and acknowledgement form. You can acknowledge all your plans at the same time on one form. To find the form, go to Administration > My Co-op’s Insurance > Insurance Plan SPD and SMM Distribution on the Employee Benefits website.

Special reminders
- A list of SPD and SMM sections that have changed for 2014 is on the Employee Benefits website
- If you didn’t distribute SPDs for one of your plans because it has no participants, fill in zero (0) on the form for the number of participants
- Complete the Number of Participants Who Are Not Literate in English section of the acknowledgement. This number is separate from the Number of Participants Enrolled, which refers to all participants enrolled in the plan regardless of English literacy.
