Use Updated HIPAA Forms Now Available Online
Updated HIPAA forms are now available on the Employee Benefits website for use by participants in NRECA health plans and programs. The forms were revised to streamline the HIPAA request process and facilitate ease of use. Specifically, the new HIPAA Authorization and Records Request Form consolidates the following retired forms:
- Protected Health Information Access Request Form
- Protected Health Information Claims Report Request Form
- Protected Health Information Authorization and Request Form–Appeal–3rd Party
Participants can access this form and all other updated HIPAA forms online by going to cooperative.com > My Benefits > Education & Resources > Insurance Plan Documents > All Plans.
