Updated HIPAA Forms Available Online

Updated HIPAA forms are now available on the NRECA Employee Benefits website for use by participants in the NRECA Group Benefits Program.  HIPAA forms only apply to requests regarding a group health plan (i.e. medical, dental and vision plans). These forms allow participants to request a claims report, request restrictions on the use and disclosure of their protected health information, submit a privacy complaint and more.

Be sure to replace previous versions of the forms that you may have saved with these updated forms, and notify your participants about the new versions, as well.

HIPAA forms are available on the HIPAA Resources page on the Employee Benefits website. Participants can find the updated HIPAA forms by going to cooperative.com > My Benefits > Education & Resources > Insurance Plan Documents, and selecting “All Plans.”
