Update: Remaining Retiree HRA Documents Available Online

The five additional documents your cooperative needs to adopt the Retiree Health Reimbursement Arrangement (HRA) administered by CBA, including instructions for completing the plan document* and summary plan description (SPD),* are now posted to the HRA – What You Need page on the Employee Benefits website. You can also access the documents from the Retiree Insurance Solutions page.

The five documents are:

  • 2015 Retiree HRA Instructions for Plan Document and Summary Plan Description
  • 2015 Retiree HRA Covered Expenses*
  • 2015 Direct Deposit Form
  • 2015 Retiree HRA Reimbursement Form
  • Sample Certification for Plan Sponsor
