Update on Outstanding Group Plan Document Acknowledgement Request

This morning, an email was sent to benefits administrators at cooperatives with outstanding acknowledgements for one or more group benefits plan disclosure notices: summary plan descriptions (SPDs), summaries of material modifications (SMMs), and summaries of benefits and coverage (SBCs). You received this email only if your cooperative has outstanding acknowledgements requiring action.

SPDs, SMMs, and SBCs that require acknowledgement are shown on the Group Plan Documents (SBC, SMM, SPD) page of the Employee Benefits website under the header Documents Requiring Acknowledgement. Each notice listed links to the online acknowledgement form. However, some or all of those may not currently be displayed for you to process.
