Deadline Is October 9 for Returning 2015 Side Accounts Forms

The deadline for returning certain forms to Cooperative Benefit Administrators (CBA)—those that are required for your cooperative to offer CBA-administered tax-advantaged side accounts to employees in 2015—is tomorrow, October 9.

Other documents should be executed and kept at your co-op and/or distributed to participants. Please use the following information as a guide.

For 125 Plans:
These forms must be completed and returned to CBA:

  • Restated 125 Plan Adoption Agreement
  • 125 Plan Employer Payroll and Funding Form
  • 125 Plan Administrative Services Agreement

Review the Checklist for Adoption of the Restated 125 Plan for instructions regarding other documents and forms.

For Health Reimbursement Arrangements (HRAs):
These forms must be completed and returned to CBA:

  • Restated HRA Plan Adoption Agreement
  • HRA Payroll and Funding Form
  • HRA Administrative Services Agreement

For the Retiree HRA:
These forms must be completed and returned to CBA:

  • Retiree HRA Adoption Agreement
  • Retiree HRA Employer Funding Form
  • Retiree HRA Administrative Services Agreement

Forms can be sent via overnight service to: Cooperative Benefit Administrators, Flexible Spending Accounts, 8701 Firethorn Lane, Lincoln, NE 68520. They can also be faxed to 402.483.9388.
