2015 Compliance Questionnaire and Resource Guide Now Available

The 2015 retirement compliance questionnaire is now available to download from the Employee Benefits website. The questionnaire asks for data about your cooperative and is due no later than February 16, 2016. W-2 salary data accompanies your questionnaire and is also due on February 16. A Message Center alert containing the 2015 questionnaire and accompanying resource guide was also sent to all primary benefits administrators on December 4.

The questionnaire has been updated for 2015 plan year reporting; however, other content changes are minor. The resource guide has also been updated for 2015. It contains the usual timeline, frequently asked questions, preparation checklist, and detailed instructions for how to send your co-op’s W-2 data. NRECA’s systems will be ready to receive W-2 data on January 4, 2016.

The questionnaire remains in PDF format. BAs can either fill it out electronically or print a copy and complete it by hand. If you plan to fill it out electronically, remember to download Adobe Reader for free if you don’t already have it. If you fill out the questionnaire by hand, enter your co-op’s name and RUS number on the second page of the questionnaire, and then print it out. Your entries will populate at the bottom of all remaining pages, and you will not need to write your co-op’s name and number on every page.
