125 Plan Sample Documents Available Online
Co-ops planning to offer a CBA-administered 125 Plan in 2015 can find restated sample plan documents posted on the NRECA Employee Benefits website, under “125 Plan 2015 Forms.”
Please note: Documents required for the 125 Plan restatement process must be completed and certain documents returned to CBA by October 9, 2014. To confirm which documents should be sent to CBA, please refer to the Checklist for Adoption of the Restated 125 Plan.
Please also note that your co-op will need a separate set of documents for each subgroup it administers, and during Annual Enrollment, employees should be enrolled under their respective subgroups.
2014 carryover adoptions
For co-ops adopting the carryover feature for the 2014 plan year—which can be done only if your co-op does not currently offer the grace period for its Health FSAs—the following documents should also be updated by October 9:
- 125 Plan Amendment – 2014 Health FSA Carryover
- 125 Plan Board Resolution – Adoption of 2014 Health FSA Carryover
- 125 Plan SMM – 2014 Health FSA Carryover (should be distributed to participants)
Look for these documents under the heading “125 Plan 2014 Forms,” on the same page.
Co-ops adopting the carryover for 2014 also need to complete and return the specified restated plan documents for the 2015 plan year.
