125 and HRA Plans: The Documents You Need for 2023 Are Now Online

For cooperatives offering the 125 Plan or a health reimbursement arrangement (HRA) plan in 2023, the sample plan documents, guides and forms you’ll need for the next plan year are now available on the Employee Benefits website.

Keep Friday, September 30 on your calendar—that’s the deadline for sending completed plan documents to Cooperative Benefit Administrators (CBA). Getting your documents returned on time will ensure that your plans are ready for open enrollment, which runs November 1–30.

Why you should download new documents for 2023
Periodic updates are made to the forms and resources, so be sure to view and complete the most current versions possible to avoid having to resubmit forms. Visit the 125 Plan and HRA pages and scroll down to Actions and Resources to get the materials you need.

Which documents do you need?
Note: If your co-op sponsors a 125 Plan, whether you are making changes to your plan or not, you must submit a payroll calendar each year for each subgroup administered. Pay dates in the payroll calendar must all be in the 2023 calendar year.
