Coming Soon: A Redesigned EOB Statement

Understanding how medical coverage applies—or doesn’t—can sometimes be a challenge for plan participants. The explanation of benefits (EOB) statement, sent each time a claim is filed, breaks down amounts paid by insurance and how much the participant may owe. But even EOBs can be hard for some to decipher.

Starting next week, the EOBs your participants receive for their medical, dental and/or vision benefits will have a new, easier-to-read format. On page 1, the updated EOB shows a summary of key amounts, including the amount billed, how much the plan paid and how much the participant may owe—with details provided later in the document.

The new EOB also provides a bar chart to help people visualize how close they are to meeting their annual deductible(s), both in-network and out-of-network.

Customized delivery
EOBs are mailed to participants’ address of record unless they have chosen to receive them electronically. They can change their preferred delivery method on the Employee Benefits website at cooperative.com > My Benefits, by clicking Profile at the top-right of the page, then Preferences. EOBs for specific claims are also available to view or download from the Claims page.
